Medicare requires a Statement of Ordering Physician. This should be completed, signed and dated by the treating physician and must be kept on file by the supplier. This form should be made available to the Durable Medical Equipment Regional Carrier (DMERC) on request. The Statement of Ordering Physician may act as a substitute for a written order if it contains all of the required elements of an order.
