Patients with morbid obesity are at risk of developing systemic complications that can affect the outcome of surgery. This case study reports the use of a method of wound closure using vacuum suction to treat a large sacral pressure sore in a morbidly obese patient. Chronic wounds such as pressure sores and venous leg ulcers are characterized by slow or absent granulation response. One approach to management involves debridement and flap closure in conjunction with pressure area care. Vacuum assisted wound closure has been reported to stimulate granulation response and reduce bacterial colonization in vitro. Its application to chronic wounds may stimulate rapid granulation response and epithelialization, allowing healing by secondary intention as an alternative to flap closure.
